Four wheels, one at a time

A car has four wheels. One flat tyre, and it doesn’t matter how well the other three are turning. A chronic condition works the same way.

Most programs ask you to fix everything at once - diet, sleep, stress, exercise, together. Change everything together and it collapses together. That’s the yo-yo cycle.

We work the four wheels in sequence, under Dr. Roshani’s supervision. Each one makes the next one possible.

Nutrition

Always first.

What we work on - Protein at every meal: 25g at breakfast, lunch, and dinner. The typical Indian morning is idli, dosa, upma, poha, paratha, cornflakes, muesli, juice - almost all carbohydrate. We add protein first. We don’t take food away.

What patients notice, within days - Full after a meal, for the first time in a long time. Sleep improves. Cravings drop. The carbohydrate conversation gets easier on its own.

Sleep

Follows, once nutrition improves.

What we work on - A fixed bedtime and wake time. Seven hours, waking fresh. Cut a healthy person’s sleep to four hours a night and you can produce pre-diabetes in a week - from sleep deprivation alone, in a published study.

What patients notice - Ghrelin, the hunger hormone, drops. Less late-night eating, less morning fatigue. What looked like a willpower problem turns out to be under-sleeping and under-eating protein.

Stress

Easier on a slept, fed body.

What we work on - The real needs underneath the stress: practical, emotional, sometimes deeper than glucose. A mind-body specialist works with patients here. Chronically high cortisol worsens insulin resistance, wrecks sleep, and drives cravings - speeding up the exact dysfunction we’re undoing.

What patients notice - Small things stop setting them off. More bandwidth for what matters.

Exercise

Last. And the most natural.

What we work on - Resistance training first. Muscle pulls 60–90% of your post-meal glucose - the more you have, the better your body handles carbohydrate, independent of medication. Ten to fifteen minutes of large-muscle work can take blood glucose from 200 to 120–130.

What patients notice - They start asking for it. In thirteen years, Dr. Roshani has never had to push a patient to exercise once the first three wheels were turning.

The direction is always toward less

In India, Dr. Roshani manages your medications directly. Internationally, she gives clinical guidance you take to your local prescriber. Either way the direction is the same - toward less, never more - as the four wheels create the conditions that make reduction possible.

Fasting comes last, if at all

Fasting is not step one. We use it only after the four wheels are stable, and only when there’s a clear metabolic or fat-loss reason. The car has to be running before you load the roof.

What this looks like in practice

Before your first call, Dr. Roshani’s team spends 30+ minutes on your file - history, symptoms, medications, reports, labs - through the Reisaan app. So when the call starts, no one is taking down information. The plan you leave with is built on your biology, your food, and your life - not a standard protocol.

Your First Consultation is 30 minutes with Dr. Roshani and your health coach. Your coach maps where you are and where you want to go; Dr. Roshani reviews the clinical picture and builds the roadmap.

When the program ends, you understand your body well enough to keep going on your own. Not dependent on a chart. Not waiting for the next appointment. One patient asked to re-enroll after finishing. Dr. Roshani said no - not to discourage her, but because she didn’t need it anymore.
